FAIL SAFE MODE

Hi I have a 2002 V8 3.2 just recently the gear shift became stiff and it struggled to move up through the gears. I drove it today after letting it warm up the gear shift was still stiff but it changed gear fine. When I parked it up for half an hour it wouldn’t start and the fail safe mode came up. The gearbox was flushed out and refilled about 18 months ago so I can’t see that been the problem, could it be low level or something more sinister?
