The workshop manual lists 12 possible causes of failure to upshift from 2-3. The only listed cause I can see that is outside the gearbox is "Manual linkage incorrectly assembled or adjusted".

Six months is an awfully long time. Perhaps it may be time to take the car elsewhere?

Persistent fast idle sounds like an air intake leak, or the throttle not returning to fully close the butterflies. I would check the inlet manifold etc for air leaks and the throttle return setting.
